Funding-Limited Submission – Health Resources and Services Administration (HRSA) – National Center for Family/Professional Partnerships (HRSA-13-206); April 1, 2013

 

Description: This announcement solicits applications for a cooperative agreement for the National Center for Family/Professional Partnerships.  The Family Professional Partnerships Program promotes the following objectives to improve the health delivery system and quality of life for children (and youth) with special health care needs (CSHCN) and their families: (1) family-centered care, (2) cultural and linguistic competence, and (3) shared decision-making for families of CSHCN at all levels of decision-making (individual, peer, community, etc.).

 

The FY 2013 priority needs are:

  1. Full implementation of the 2010 Patient Protection and Affordable Care Act, such as providing technical assistance around

·         Title III-Improving Quality & Efficiency of Health Care / Sec. 3502. Establishing community health teams to support the patient-centered medical home.

·         Partnering with Medicaid to outreach to underserved populations eligible for medical assistance including CSHCN, racial and ethnic minorities, rural populations.

·         Families partnering with Health Insurance Marketplaces (Exchanges).

·         Families partnering with community networks (i.e., community transformation grants, collaborative care networks, Primary Care Extension Hubs, etc.).

  1. Strengthening the primary care workforce, such as:

·         Training and education for states, providers and family leaders.

·         Mentoring approaches.

·         Tools and curricula.

  1. Improving access to quality care and innovation in care and services, such as: 

·         Outreach and enrollment.

·         Removing barriers to access, especially Tribal, Hispanic and rural populations.

·         Measuring impact.

·         Quality improvement.

 

Amount: Approximately $475,000 is expected to be available annually to fund one awardee. The period of support is three (3) years.

Institutional Limit: Multiple applications from an organization are not allowable.

 

Internal Competition:  Because HRSA limits the number of proposals to one, the University of Kentucky has established an internal selection process.  Prospective applicants interested in responding to this program should send a pre-proposal with the following information to the Office of the Vice President for Research at limitedsubmissions@uky.edu and a copy to their Associate Dean for Research by Monday, April 1, 2013:

  1. Names and departments of the Project Director, project personnel and partners;

2.     A brief abstract of the proposed project including the goals and objectives, activities, coordination between agencies, and evaluation (2 p. max).
