Submission – Administration for Community Living - PPHF - 2014 - Evidence-Based
Falls Prevention Programs Financed Solely by 2014 Prevention and Public Health
Funds; June 4, 2014
Purpose: This funding
opportunity is designed to increase the number of older adults, American
Indian, Alaskan Native and Native Hawaiian elders, and adults with disabilities
at risk for falls who participate in evidence-based community programs to
reduce falls and falls risk, while concurrently
increasing the sustainability of these programs through innovative funding
arrangements. The awards will be made in the form of cooperative agreements
because the Administration on Aging (AoA) will be
substantially involved. These cooperative agreements have an anticipated start
date of September 1, 2014.
Funding
Amount: This
funding opportunity has two components, state and tribal grants, with differing
funding levels. The Option 1 state grant awards will support approximately 8-10
grants to domestic public or private non-profit entities averaging about
$400,000 per award, for a two-year forward funded project period. The Option 2
tribal grant awards will support approximately five grants to Federally
Recognized Tribes and Tribal Organizations up to $100,000 per award for a
two-year forward funded project period.
Institutional
Limit: The
University of Kentucky may submit only one application for Option 1.
Internal
Competition: Because
this sponsor limits the number of applications that can be submitted, the
University of Kentucky has established an internal selection process.
