Can
you develop a digital solution to help providers and/or patients connect to
services related to social determinants of health?
Social
determinants of health (SDoH) such as education,
employment, transportation, and community and social norms, can have critical
impacts on well-being. However, these factors are often overlooked and excluded
in established health care frameworks. Technology has the potential to improve
how we address ongoing care delivery by utilizing SDoH information to provide targeted, personalized, and
comprehensive care that accounts for the drivers of health in
communities.

& Community Based Care Innovation Challenge
Can
you create a technology that supports the advancement of at-home or
community-based health care?
Digital
solutions empower consumers and providers to actively engage in managing and
improving health outside of traditional clinical systems. New technologies
provide convenient, affordable, and comfortable care within homes and
communities. This movement can greatly support individuals of all ages across a
variety of geographies. However, despite these benefits, the US continues to use
mostly institution-based care models. Cultural shifts and more innovation is
required to give individuals, caregivers, and providers the tools they need to
address healthy living within the home and community.
Amount
Each
competition will have Phase I: Submit tech-enabled solutions. Phase II: 5
semi-finalists will be awarded $5,000. Grand prizes:$40,000 (First
place), $25,000 (Second place), $10,000 (Third
place)
